What is Premarin Cream?

Premarin cream contains a mix of estrogen hormones. Estrogen is a female sex hormone which is produced in by the ovaries and is necessary for many body processes. Premarin vaginal cream is prescribed by your doctor to treat symptoms of menopause such as vaginal dryness, burning or irritation as well as hot flashes and painful sexual intercourse. These symptoms are caused by the body making less estrogen and the medication works by replacing the estrogen that is normally produced by the body. As well as being prescribed for menopause, Premarin cream may also be used to treat kraurosis vulvae (a condition that may cause vaginal dryness and discomfort in women or girls of any age).

How to use Premarin Cream

Premarin vaginal cream is usually applied on a schedule alternating multiple weeks where the cream is applied daily, and then one week when the cream is not applied at all. Always follow the instructions prescribed by your doctor. When using Premarin cream, it should be used at around the same time every day. When using, you should fill the applicator with the correct amount of cream and then lie on your back with your knees up toward your chest. Gently insert the applicator into your vagina and press the plunger down to release the cream. Slowly remove the applicator from your vagina and clean the applicator with mild soap and warm water.

  • Premarin vaginal cream is usually applied on a schedule alternating multiple weeks where the cream is applied daily, and then one week when the cream is not applied at all.
  • Remove the cap from the tube of cream.
  • Screw the applicator’s nozzle end onto the tube of cream.
  • Gently squeeze the tube from the bottom to fill the applicator with the correct amount of cream. Use the markings on the applicator to measure your dose. Unscrew the applicator from the tube.
  • Lie on your back and pull your knees up toward your chest.
  • Gently insert the applicator high into your vagina, pressing the plunger to release the cream before slowly removing the applicator from your vagina.
  • Clean the applicator by removing the plunger from the barrel and wash both the applicator and the plunger with mild soap and warm water. Do not use hot water for washing the applicator.
  • If you miss a dose of this medication, use it as soon as you remember. If it is almost time for your next dose, skip the missed use. Do not use extra cream to make up for the missed dose.

Cautions

Make sure to tell your doctor and pharmacist:

  • If you have any drug allergies.
  • Which other prescription and non-prescription medications you currently take.
  • If you have ever had a hysterectomy.
  • If you have ever had heart disease, liver or kidney problems or gallbladder disease.
  • If you have ever had asthma, epilepsy, seizures, migraines, lupus, endometriosis, hereditary angioedema porphyria, a thyroid disorder or high levels of calcium in your blood.

Also be aware that:

  • You should not take this medication if you are pregnant. Inform your doctor if you think you may be pregnant.
  • Premarin cream can weaken the latex of a condom, diaphragm or cervical cap. Talk to your doctor about the best birth control methods to use while taking this medication.
  • You should not use this medicine if you have unusual vaginal bleeding, liver disease, a history of heart attacks, strokes or blood clots or have an increased risk of blood clots because of a heart or blood disorder.
  • You should not use this medication if you have a history of breast, uterus or vaginal cancer.
  • Premarin cream can increase your risk of blood clots, stroke or heart attack. You are at even more risk if you have high blood pressure, diabetes, high cholesterol, or if you are overweight.
  • Do not smoke or use tobacco. Estrogens combined with smoking increases your risk of stroke, blood clots, high blood pressure or heart attack, especially in women over 35.
  • This medication may cause blotchy, dark areas on your skin or face and sunlight may worsen this effect. Limit your time in the sun and wear sunscreen or protective clothing when outdoors. Avoid tanning booths and sunlamps.
  • You should examine your breasts every month and have a mammogram and breast exam performed by a doctor every year.  

Side Effects

Side-effects may include but are not limited to:

  • Breast pain or tenderness.
  • Nausea, heartburn or vomiting.
  • Dizziness, nervousness, depression or irritability.
  • Blotchy or spotty darkening of the skin or face.
  • Difficulty falling asleep or staying awake.
  • Changes in sexual desire.
  • Hair loss or unwanted hair growth.
  • Sudden feelings of heat or sweating
  • Difficulty in wearing contact lenses.
  • Swelling, redness, burning, itching or irritation of the vagina, vaginal discharge or difficult urination.
  • Cold or flu symptoms or back pain.

Contact your doctor if you experience any of the following side-effects:

  • Bulging eyes or yellowing of the skin or eyes.
  • Pain, swelling or tenderness in the stomach.
  • Loss of appetite or weakness.
  • Joint pain or movements that are difficult to control.
  • Rashes, blisters, hives or itching.
  • Swelling of the face, throat, hands, arms or other body parts.
  • Hoarseness or difficulty in breathing or swallowing.
